SOP Training Records: Proving Who Read Which Version

An SOP training record shows that a named person read, or was trained on, a specific version of an SOP on a specific date. The record can be a read confirmation, a quiz result or an electronic signature. When the SOP changes, the people who follow it need a new record for the new version.

What should a training record contain?

  • The person's name or user account.
  • The SOP ID and the exact version number.
  • The date and time of the confirmation.
  • How it was confirmed: a click, a quiz score or a signature.
  • Who assigned it, if it was assigned.

Which method should you use?

Match the method to the risk. A read confirmation is enough for low-risk office procedures. A short quiz shows understanding, which matters when a mistake is costly. An electronic signature is the strongest record and is expected for many regulated procedures; for FDA-regulated records, Part 11 sets the controls a signature must meet.

What happens when the SOP changes?

A record for version 3 does not cover version 4. Good practice is to reassign the SOP to everyone who follows it when a new version is approved, and to track who has not yet confirmed. What should you report on?

Three reports cover most needs: who has not yet confirmed the current version of each SOP, which people hold a confirmation only for an older version, and training completion by role or team. Run them after every approved change and before every audit.
